Coronary physiology is performed in order to: (a) assess the hemodynamic significance of native coronary lesion and determine the need for PCI; and (b) to assess whether an optimal result was achieved after PCI. Both hyperemic (usually adenosine FFR) and non-hyperemic indices (such as iFR, dPR, RFR) can be used, with non-hyperemic indices used more commonly as they are easier to perform. Coronary physiologic assessment is performed in 8 steps, as follows: 1. Flush + zero pressure wire 2. Insert pressure to tip of guide 3. Remove introducer, flush guide with NS, equalize pressures 4. Advance pressure wire distal to the target lesion 5. Measure resting physiologic index 6. Measure hyperemic FFR (if needed) 7. Pullback 8. Check for drift

Discussion on fractional flow reserve or FFR for guiding coronary percutaneous intervention and functional SYNTAX score. Coronary angiography gives a visual impression about the severity of the stenosis. But it need not imply the actual functional significance of the stenosis in terms of flow physiology. It is often difficult to decide which are the flow limiting lesions when there are multiple stenoses in same or different territories. It is here that the fractional flow reserve estimation helps. FFR is estimated using a guide wire with a pressure transducer. FFR wires have properties similar to the floppy guide wires so that they can passed across coronary lesions back and forth easily to assess the pressure drop across the lesions.
Fractional flow reserve (FFR) wires are notoriously difficult to maneuver. We discuss a quick and simple technique to pass FFR wires across difficult lesions with a workhorse already in place using aspiration thrombectomy catheters. Philip McCarthy, United Kingdom, discusses the state of coronary physiology today with Philippe L’Allier, Canada. The results of the landmark studies DEFER, FAME and FAME II have all shown that using adjunctive physiology not only improves the outcome of intervention but also decreases stent usage, stops unnecessary stent deployment and increases contrast usage which makes us end up with a more refined technique. However, despite the advantages to the interventionalist day-to-day, the process has been slow – McCarthy explains why, how this can change, and also discusses the future of the field. 42-year old male with CCS Class II angina and coronary CTA revealing 3 V soft coronary plaques. A Cardiac Cath on February 25, 2022 revealed 3 V CAD: 90% RCA-AV Continuation, 90% LAD-D1, multiple 80% mid LAD, 95% LCx-OM2 bifurcation with normal LV function and SYNTAX Score of 22. Patient underwent successful interventions of RCA-AV Continuation and LCx-OM2 using two Promus Elite DES. Patient is now planned for staged PCI of FFR guided intervention of multiple LAD/Diagonal lesions.
